What it is

L-glutamine is a conditionally essential amino acid, the most abundant free amino acid in blood and skeletal muscle. The body synthesises it readily, and dietary protein supplies substantial additional amounts.

Its conditional essentiality matters. Under severe physiological stress such as burns, major surgery, sepsis or prolonged critical illness, demand can outstrip supply and plasma glutamine falls. Supplementation in those settings has real evidential support.

The gap between that clinical picture and a healthy adult eating adequate protein and training four times a week is enormous. A meaningful portion of glutamine's supplement reputation comes from applying critical care findings to a population that does not share the deficit those findings addressed.

How it works

Glutamine is the preferred fuel for enterocytes, the cells lining the small intestine, and for rapidly dividing immune cells including lymphocytes. This underpins both the gut health and immune applications.

It participates in nitrogen transport between tissues, in acid-base balance through renal ammonia production, and as a precursor to glutathione synthesis. It also contributes to cell volumisation, which has been proposed as a signal for protein synthesis.

The obstacle for the muscle-building case is first-pass metabolism. A large share of oral glutamine is consumed by the gut and liver before reaching systemic circulation, so raising muscle glutamine content through supplementation is considerably harder than the plasma data suggests.

What the evidence shows

For muscle growth, strength and body composition in healthy trained individuals, controlled trials have repeatedly found no meaningful benefit over placebo when protein intake is adequate. This is a reasonably consistent negative finding rather than an open question.

Gut barrier research is more interesting. Trials in intestinal permeability, including in athletes exercising in heat, have reported protective effects, and there is a body of work supporting use in specific gastrointestinal conditions.

Immune findings in endurance athletes are mixed. Some older work reported reduced infection rates after prolonged exertion; more recent and better-controlled trials have generally not replicated this. Clinical evidence in critical illness and burns remains the strongest part of the literature, and the least relevant to most people buying it.

Dosing

Common supplemental intake is 5-10 g daily. Gut-focused protocols sometimes use up to 20-30 g daily split across servings, reflecting the doses used in clinical research. If the goal is muscle growth, the honest advice is to spend the money on total protein instead. A whey serving supplies several grams of glutamine alongside a complete amino acid profile, and the evidence for that is far stronger. Where it may be worth using is gut barrier support, particularly around prolonged endurance work or heat stress. Take it away from other protein sources for that purpose, since amino acids compete for transport.

Safety and side effects

Glutamine is well tolerated, with a strong safety record across a wide dose range including high intakes used in supervised settings. Adverse effects are uncommon and generally limited to mild digestive discomfort.

Its metabolism generates ammonia, which a healthy body clears without difficulty. Anyone whose liver is not functioning normally should not take supplemental glutamine without medical guidance.

Long-term data at supplemental doses is limited. As with any supplement, anyone under medical care or taking regular medication should check before starting it.
