What it is

Glucosamine is an amino sugar produced naturally in the body, where it is used to build glycosaminoglycans, the molecules that give cartilage its structure and its ability to hold water.

Commercial glucosamine is usually derived from shellfish shells, which makes allergen declaration mandatory and matters to a meaningful number of customers. Vegetarian versions produced by fermenting corn are available and are chemically identical.

Two forms dominate. Glucosamine sulphate, particularly the crystalline potassium-stabilised version, is the form used in most of the trials that reported positive findings. Glucosamine hydrochloride is cheaper, contains more glucosamine by weight, and has performed noticeably worse in trials. If you are going to take it, this distinction is the one that matters.

How it works

The proposed mechanism is straightforward substrate supply: glucosamine is a precursor for glycosaminoglycans, so supplying more is meant to support cartilage maintenance.

That logic has an obvious weakness. The body already makes glucosamine, and there is no evidence that its production is the limiting factor in cartilage turnover. Supplying more of something that is not in short supply rarely achieves much.

An alternative explanation is that glucosamine has anti-inflammatory effects independent of its role as a building block, with reported influence on inflammatory signalling in joint tissue. The sulphate portion may also matter, since sulphur is required for cartilage synthesis, which would explain why sulphate outperforms hydrochloride.

What the evidence shows

The evidence is genuinely mixed and has been argued over for years. Trials using crystalline glucosamine sulphate have more often reported improvements in joint comfort and function than trials using the hydrochloride form, which have largely been negative.

Large independent trials have generally found effects close to placebo, while industry-funded trials have tended to report benefit. That pattern is itself informative and is a fair reason for caution.

For healthy active people using it preventively, there is essentially no evidence. Most research involves older adults with established joint complaints, and it does not transfer to a lifter hoping to protect their knees. Effects, where reported, take several weeks to months to appear.

Dosing

The standard dose is 1,500 mg daily, used with remarkable consistency across the research. Some protocols split it into three 500 mg servings, others use a single dose. Choose crystalline glucosamine sulphate over the hydrochloride form. The price difference is small and the evidence difference is not. Take with food. Give it at least eight to twelve weeks before judging whether it is doing anything, since effects in trials accrue slowly. It is often combined with chondroitin and MSM, though evidence that the combination outperforms glucosamine alone is weak.

Safety and side effects

Glucosamine is well tolerated across a large body of research. Digestive upset, nausea and heartburn are the most commonly reported effects and are usually mild.

Shellfish-derived glucosamine requires allergen declaration. Reactions are uncommon, since the allergenic proteins are in the flesh rather than the shell, but anyone with a shellfish allergy should use a vegetarian version rather than take the chance.
