What it is
Cissus quadrangularis is a perennial climbing vine native to India, Sri Lanka and parts of Africa, recognisable by its distinctly square, jointed stems. Its Sanskrit name, hadjod, translates roughly as bone joiner, reflecting its traditional application to fractures and skeletal complaints.
The plant contains ketosteroids, stilbenes including resveratrol and quercetin, flavonoids and a notable calcium and vitamin C content. Commercial extracts are usually standardised to ketosteroid content, though standardisation practices vary widely across suppliers.
In sports nutrition it occupies an unusual position: it is used both for joint and connective tissue support and, separately, in weight management formulas. These two applications draw on largely separate bodies of research and should not be treated as one claim.
How it works
For bone and connective tissue, the proposed mechanism centres on stimulation of osteoblast activity and collagen synthesis. Animal work has reported accelerated fracture healing and increased bone mineralisation, with the ketosteroid fraction generally credited.
Anti-inflammatory and analgesic activity has also been reported, which would be the more plausible route for any short-term symptomatic relief in tendon or joint discomfort, as distinct from structural repair.
The weight management rationale is separate and considerably weaker, resting on proposed effects on lipase activity, appetite signalling and glucocorticoid metabolism. These mechanisms are largely preclinical and should be treated with caution.
What the evidence shows
The preclinical literature on bone healing is reasonably extensive and consistent in direction. Human evidence is much thinner. A small number of trials have examined joint pain in athletes and reported symptomatic improvement, but sample sizes were small and durations short.
The weight management trials warrant particular scepticism. Several frequently cited studies share overlapping authorship and funding, and results have not been independently replicated at scale. This is a recognised pattern in botanical supplement research and a fair reason to discount the findings until better data exists.
The reasonable position is that Cissus has a long traditional record and a plausible mechanistic basis for connective tissue support, but the human evidence does not yet justify confident claims in either application.
Dosing
Doses used in trials and in practice vary widely, from around 500 mg to 3,200 mg daily, reflecting the lack of formal dose-ranging work.
Joint and connective tissue protocols typically use 1,000-3,200 mg daily of extract, usually split across two or three servings and taken with food.
Dose against declared ketosteroid content where the extract is standardised. Given the range in extract strength across suppliers, milligram comparisons between products are close to meaningless without that figure. Effects reported in trials generally accrue over 8 weeks or more.

Safety and side effects
Generally reported as well tolerated in the available trials, with mild gastrointestinal effects such as flatulence, dry mouth and headache being the most common complaints. It has a long history of traditional use, which offers some reassurance about acute safety.
Long-term human safety data at supplemental doses is limited, and the interaction profile has not been thoroughly characterised. There is some evidence of effects on blood sugar, so take care if yours is being managed.
